Guidelines to Add an Integrated Institution (Surgery and Thoracic Surgery)
The following documentation must be submitted:
- A summary statement that includes the:
- educational rationale;
- educational objectives;
- proposed date of implementation;
- duration of the assignment;
- comment on issues identified at the last site visit;
- the distance from the primary clinical site;
- identify where conferences will occur
- a current block diagram and the proposed block diagram of a typical resident's assignments;
- the operative data for the proposed institution substantiating that the number and kinds of patients available for education will support the educational rationale;
- a letter of support for the proposal from the DIO
- a copy of the complete integration agreement
- CV of integration site program director
Submit all documentation in one package, please.
