The CI "Story": The ACGME Committee on Innovation
At its September 2004 meeting, the ACGME Board of Directors authorized the formation of the ACGME Committee on Innovation (CI), at the request of the Subcommittee that oversaw the implementation of the ACGME Common Duty Hour Standards.
At its sunset in September 2004, the ACGME Duty Hour Subcommittee recommended the formation of an ad hoc Committee, with a broad charge to explore the factors that contribute to innovation and improvement in the learning environment at the program and institutional levels. The aim was to avoid the undesirable consequences of treating duty hours as a stand-alone issue, separating it from other standards programs important to achieving high quality resident education and safe and effective patient care. Another goal entailed exploring ways to promote a high-quality learning environment, including innovation and re-engineering the clinical-education interface, and explore how the general competencies can be applied to resident education and evaluation, and their role in promoting innovation and improvement in the learning environment. Members include ACGME and public directors, residents, program and institutional leaders, and researchers at the interface of education and clinical care. Activities encompass the collection and interpretation of information on the relationship between duty hours and other aspects of the learning environment, focus groups to explore the perspective of residents, exploration of whether existing ACGME standards could be used to stratify performance above a minimum compliance level, and identify the factors that contribute to excellence in the learning environment. The committee also seeks to apply industrial and human factors engineering principles to the learning environment. Associated work entails collecting and disseminating information on notable practices that enhance the environment for residents, and exploring ways to share ideas and collaborate with other organizations in graduate medical education.
The ACGME Board of Directors authorized CI for a period of five years, with the option to extend the charge to facilitate long-term learning about the effect of the duty hour standards. CI actively collaborates with the ACGME’s standing Monitoring and Program Requirements Committees, the Council of Review Committee Chairs and other organizations in medical education to collect and evaluate information on the effect of its duty hour standards, and explore ways to refine its standards and compliance mechanism and to maximize organizational learning about the duty hour limits and their role in creating an improved learning environment for residents to contribute for “good learning for good healthcare.” The ACGME Board endorsed the recommendation of the Duty Hour Subcommittee that future changes and refinements to the common duty hour standards be based on information about their positive effect on patient care, resident learning and resident safety and well-being.
In its three years to date, the committee has undertaken a variety of activities in meeting its charge, including focus groups of residents, program directors, Review Committees chairs and other, and exploring a broad range of sources on innovation and improvement. In September 2007, the ACGME Board of Directors endorsed for filing the first formal report of CI.
